I think this occured during an alt-tab operation. If I can track down exactly when this error occured or how to reproduce, I'll post again.

I am running:
XP Professional SP2
Dual-Monitors
TaskSwitchXP Pro 2.0.11 <-- this may play a part since it crashed during an alt-tab

Is there any additional information that would be helpful?

Ok...it does appear to be TaskSwitchXP Pro 2.0.11's fault. The error occurs when it is running and doesn't occur when it is not.

BTW, Ultramon 3.0 Beta 1 worked with this application.

I've been able to reproduce the issue, will look into this for the next release. Most likely there will be a Beta 2 update within the next couple of weeks.

I get the following new crash on Beta 2, whenever I hibernate my laptop:

Ultramon encountered a fatal error and will
exit. This usually happens when the system is
low on memory or othe resources, but might also
happen for other reasons.

Actually it happens just after the hibernated laptop restarts...

So far I've seen no problems with resuming from standby or hibernation, which version of Windows are you using, and what applications do you have running?
