Just encountered an issue. Win 7 x64.
The smart taskbar, which I have set to auto-hide and to always be on top, did not appear (unhide) when it should have. This happened on the two monitors on which I have the smart taskbar configured that way (auto-hide and to always be on top).
Normalizing a maximized application on the affected monitors fixed the issue, even after re-maximizing the application it behaved normally again. So at least I know what to do when the smart taskbar stops appearing when it should.

Did you notice anything which seems to cause the problem? I haven't been able to reproduce this issue so far.

It is probably Win7 that causes this, since I have the same situation with RocketDock and maximized windows.
It actually shows, but behind the maximized window, so it seems that Win7 "move-to-top" a maximized window sometimes.
