Somehow we have missing buttons on one of our XP computers.
I also tried some of the following by reading the forums:
1) Unselected menu buttons reselect 2) Switch to XP basic theme 3) Confirmed not in compatibility mode 4) Uninstalled version 2 and installed 3.0.10. Released Feb 15, 2010

Are the buttons missing from all applications, also Internet Explorer or Notepad?

No, cmd prompt they do not appear as well.

I'm not sure what would cause this, are you sure the buttons are enabled under UltraMon Options > Buttons and Menu Commands?

Absolutely positive they are enabled. I've also tried unchecking and rechecking the checkboxes.
