I have a blue box with white text on my 2nd monitor that says "Out of Range." It's never happened before, what does this mean?

Sounds like an error message from some application, doesn't seem to be from UltraMon though.

BTW, if this message takes up the whole screen, then it's an error message from the monitor meaning that the currently selected refresh rate or display mode isn't supported.
To fix this, change the display settings for the monitor.
