First time I've ever seen this and I've installed UltraMon on something like 50 systems for myself, clients and friends.
It's Win7 x86, UltraMon 3.0.10. When the SmartTaskbar is enabled, there are no active icons in any taskbar - Smart or standard. Turn off the Smart Taskbar and they show up immediately. Turn it back on, icons stay but any newly opened programs don't show up.
I've even tried uninstall, reboot, reinstall.

What happens if you launch a new application, then move it to one of the monitors with an UltraMon taskbar? Does a task button get added for the application?
