Have been getting a few of these since 3.1.0 was installed:
UltraMon encountered a fata error and will exit.
More information:
Version: 3.1.0 OS: 5.1.2600 Service Pack 3 x32 Source file: .\MultimonGUI.cpp Line: 1574 Return value: 0 Last error: 0
I cannot find a causal relationship yet, but I have also been occasionally getting ghost window remains on the central monitor when changing from single to multiple monitors. Not sure if its related.
Havent put any new or updated apps on recently, but do apply all MS patches.
Running 3 monitors.
Ideas?

Are you using a custom language pack? You'll get this error if UltraMon fails to load the menu.

I would recommend repairing your installation, you can do this via Control Panel > Add or Remove Programs, select UltraMon from the list, click on the support link, then click on Repair.
If that doesn't fix the problem, open Task Manager when you get this the next time and check if the system is low on memory (you can see this on the Performance tab), might be a resource issue.

Just take a look at the PF Usage scale, if this is at the top it means memory is pretty much maxed out.

Its been a while. Crashes happen less often so its been a bit more difficult to diagnose. Not a memory problem. It might be associated with Adobe Acrobat Pro. I an using 7.1 and have a patch installed to allow Acrobat to open multiple windows, and the crash only seems to happen after I have done that. Not always, and not often immediately after. The sequence seems to be opening multiple PDFs and then changing number of active monitor settings with a hotkey, but again, not always. I am guessing its a hook thing that is chewing kernel memory, but that's a blind stab.

UltraMon reloads the menu in response to display configuration changes, this would explain why you get the error after disabling/enabling monitors, not sure though why this would be related to having multiple PDF files open.
Does UltraMon work fine if you restart it after you get the error?

Doesn't seem to be a resource issue if it works fine again after restarting UltraMon. If this only happens with 3.1.0, which version of UltraMon were you using previously?

I can't be absolutely certain, but almost certainly the prior version.
I was thinking it might be a resources issue because rarely I would get to a point where I could not open any more IE windows. They just wouldnt. Then I would close a couple and do something else and I would get Windows "uh oh" sound and find an Ultramon error.
The problem is not happening anywhere near as often, but I have also recently upgraded to IE8, so there are probably way to many changes happening on my PC to be useful in diagnosing. Come to think of it, it hasnt happened since I installed IE8 earlier this week. Probably just a coincidence.
Perjhaps I should just let thread this go until I can be sure of some form or repeatability or causual sequence, and that might take a while. I will comment again if I can be of more use.

3.0.10 is basically the same in this regard, I don't think that this is due to something which changed in 3.1.0.
If you get the issue again where you can no longer open new windows, take a look at this thread for a fix.

More grist. After not having had one for a while a circumstance arose which might be useful. I had been running the same logon session for several days without incident. I deleted and reinstalled a printer. Immediately after I closed the completed printer installation dialogue I received:

I only had one monitor active at the time.

Not sure what would cause this if only a single monitor was enabled, but there is an issue with window buttons and multiple 16-bit applications which could cause this error. If you get this again let me know and we can do further troubleshooting.

I followed the advice in this thread as suggested and my machine is now far more stable, and I have not had UltraMon barf in two weeks. Here's hoping ...
