Dell E6500 laptop. Dell E2209W monitor.
Both running in native resolution
Neither scaling
about 20% of the time after unlocking the main monitor (external) will not display windows to full screen. The background image displays as normal (e.g. background covers 100% of screen, window covers only the left 80%). Once this occurs there is nothing that can be done to allow windows to maximise to full screen, however windows can be manually resized to fill the screen.
Once this happens I've tried changing resolution, refresh rate, scaling, enabling and disabling moitors, exiting and killing UltraMon processes. The only thing that fixes this is a reboot.
Typically the external monitor will have this issue about 20% of the time I unlock, sometimes the laptop screen will do it to.

Does the same thing happen if you don't use UltraMon at all?
Also, do you have any screen partitioning apps or display driver settings enabled?
