Often (but not always) #2 Monitor (but primary) is not recognized when loging into Windows 7 64bit. I must got to Control Panel/Device Manager/Monitor/Scan for hardware changes, then all is fine again. Any ideas?

Maybe an issue with the display driver, if you haven't done this already I would recommend installing the latest drivers for your video card.

That was a good idea, and there was an updated driver, but still experiencing the same issue. After login (and monitor hybernation) the primary (but #2) monitor is not recognized. The biggest problem here is that all icons are rearranged each time, although it is possible to re-recognize the monitor from the control panel/device manager/monitors/scan for hardware changes. ATI Radeon HD 3400 card Any other ideas?

Unfortunately I don't know what else you could try, definitely sounds like some sort of hardware issue though.
As a workaround for the icons issue, you could save/restore icon positions via UltraMon menu > Desktop Icons.

Yes, that is what I do, it just gets old after several times per day. The new 3.1.0 update is better with remembering icon positions after restoring monitors via device manager/search for new hardware, but still often looses 2nd (yet primary) monitor after restore from log off. Frustrating.
