After installing Ultramon, if I am presented with a normal "OK or CANCEL" dialogue box, the text within the box does not appear. The "?" appears inside the box, but both the title and the other text are missing. In addition, the buttons for OK And CANCEL appear, but the words do not. What did I do wrong? This happens on all dialogue boxes, but only on the secondary monitor, not the primary.

Googling around, I find this problem is not unique to Ultramon although in my case, it first started when Ultramon was installed and only occurs on the secondary monitor. It occurs with almost any dialogue box, not just OK/CANCEL. In addition, right click context menus occasionally have options blanked out. Based on other google searches, I doubt Ultramon is the culprit although none of the posts on the topic offer a remedy other than to reinstall windows. Here is the setup from the "About" box in case that might help.
UPDATE: After I created this post, I noticed that Ultramon assigned the secondary monitor with 8-bit color. After changing this to 32 bit color, the problem went away. Fascinating. Sorry for the jumbled reply. Now, here *was* the setup before I changed it.
2 monitors Current desktop: 3360x1200 (0,0 - 3360,1200)

Glad to hear the issue is fixed, unfortunately I don't know why the monitor would have been set to 8-bit color, UltraMon setup should have no effect on the current display configuration.
