Hi, I have four screens of all different resolutions and sizes.
I name them after manufacturer: Dell (1280x1024), Samsung (1920x1080), HP (2560x1600), Acer (1920x1080).
Generally, I use the HP as the main, with the Acer and Dell as secondaries to the left and right, respectively. I set up a profile for these three, like this:

In the above config I have the Samsung disabled.
Sometimes I use the TV (Samsung) as the main by itself. I have a profile for that too, with the Samsung set as the main display and all others disabled.
I like to have a shortcut for both on the desktop so I can quickly change from one to the other.
The thing is, after setting up the display profiles for the first time, it works great. Then a day or two later when switch profiles, Ultramon often confuses the screens.
For instance, it may set the Dell as "HP" and make it the main, or call the Samsung TV "Acer" and make it a secondary. These are examples. It may mix them up in other ways too.
When this happened the first few times, I just deleted the profiles and remade them. This worked for the first few times. Then the above happened again. After a few times doing this I just began to manually enable the correct screens via the task bar icon every time I changed configurations.
As you can imagine, this is not convenient. I would very much prefer to have the shortcuts work.
Is this a known issue with a quick fix?
I should note that:
I often leave my PC on for several days at a time but need to reboot every few days.
The Samsung TV is often off when changing profiles--to change to the Samsung only profile, I need to turn the Samsung on.
The Samsung is fed via HDMI like this: PC ===> Onkyo receiver ===> Samsung. A Directv box is used for the Samsung when it isn't being used for the PC, with the Onkyo acting as an HDMI switch.
Sometimes when I am on my PC with the HP as main, and I turn on the Onkyo and Samsung to watch Directv, it will automatically switch to the Samsung as PC main if I have inadvertently left the Onkyo set to PC.
I apologize for the long post, but I figured the problem may be complicated and I'd love to have it work right! Thank you!

Quick edit to my previous post:
I forgot to mention that I have all four screens plugged into the dvi ports of two Nvidia GTX 260s. Thanks in advance for any help at all!

Could be an issue with the monitor numbers, which version of Windows are you using?

Please try the following: enable all 4 monitors, then run FixMonIds.vbs.
The script will make sure each monitor has a unique number assigned. If monitor numbers were changed you may need to adjust your display profiles afterwards.
