When Ultramon starts up, i get this error: Ultramon Desktop encountered a fatal error and will exit.
Version: 3.2.1 OS: 6.1.7601 Service Pack1 x64 Source File: WallpaperUtil.cpp Line: 145 Return value: -2147287035 Last Error: 5
Windows 7 Pro x64 and this product has been running fine for the past couple of years, then all of a sudden at the end of last month this error pops up when I try to do a lot of items on the menu. Can't change my wallpaper obviously because of this, in fact when I try to change, I get the dialog box that UltraMon Desktop has stopped working. Help

You'll get this if UltraMon fails to save the wallpaper profile due to being denied access to the file. You get this during startup because UltraMon refreshes the wallpaper profile in case display settings have changed.
To fix this, open properties for the folder C:\Users\<username>\AppData\Roaming\Realtime Soft, then select the Security tab and make sure your user account has Full Control over the folder and subfolders.
Please note that the AppData folder is hidden by default, you can show it via Control Panel > Folder Options > View > Show hidden files and folders.

All directories down to the wallpaper directory are set for Full Control and the owner of the directories but still getting the error.
any other suggestions?

You could try deleting the wallpaper profile and then recreate it via UltraMon menu > Wallpaper, unfortunately I don't know why you would get this error if your user account has read/write access to the file.

Ok, that seemed to work. I deleted all the profiles and re-added them. It is a bit of a pain, but not much really,

I had the same issue where randomly I would get this error but usually a restart fixed the issue. Recently that didn't work either so I then found this post.
When I checked the properties, my user had full permissions as well. But then I noticed in the General tab, the folder was marked as Read Only. I unchecked it, restarted and all was well.
But I went to check again and for some reason the Realtime Soft file is Read Only again. We'll see if the issue persists, but at least now all is working and the fix is easy enough.
