Every time I reboot, I get a mysterious empty space in the middle of my desktop icons. Like half of my icons were moved down one position.
If I open UltraMon, click Save Position, then Restore Position, half of my icons just get scrambled and one position in the middle is left empty.

What icon settings do you have enabled? You can see this by right-clicking the desktop, then select View from the menu.

I have now tested with those icon settings and UltraMon 3.2.2, but so far haven't seen the issue.
Two things you could check:
- do you have any icons with the same name? This will cause problems because UltraMon will try to place them at the same position
- do you have another application installed which manages desktop icons, for example Fences?

Nope, I don't, still happens...it may even be deleting icons..

Please try if disabling icon positioning via UltraMon Options > General (uncheck 'preserve position of desktop icons') fixes the issue, this way UltraMon will no longer try to restore icon positions during startup.
