Running Windows 7-64 with 3 monitors, 1 disabled (laptop driving dual monitors), extended desktop with Smart Taskbar in Standard Mode. When Outlook 2007 is placed on the extended monitor, clicking on the Outlook shortcut brings up a new Outlook window, versus switching to the existing one that is already running.
The work-around is to disable the taskbar on the extended monitor

You'll get this if Outlook is pinned to the taskbar. You could switch the Smart Taskbar to mirror mode (via UltraMon Options > Smart Taskbar), this way clicking on the Outlook task on the main taskbar will activate the existing Outlook instance.
