there was a thread about desktop rotation/orientation a while ago (couldn't find it right now).
i don't know if its a standard nvidia driver feature that is disabled by default (like clock speed), but with the tweaked omega drivers http://www.omegacorner.com/ i got a tab to rotate the desktop in 90 degree steps. very much like the pivot software does.
for fun i fired up fs2002 and to my surprise it worked (damn slow tho). screenshot: http://www.gameart.com/hosted/braindead/temp/portrait.jpg

Yes, it's nowadays a stabdard feature, although it is disabled as you noted. Some other progarams sides of the Omega driver can enable the tab. Forgot which progs tho..sorry :)
