I'm seeing that Microsoft is moving to some new type of window. I don't have the UltraMon buttons for Visual Studio 2013/15 and SQL Server Management Studio 2014/16 windows. Is there a way to get the UltraMon buttons to show up in these new versions.
I'm running these on Windows Server 2012 standard addition. Had the same issue running these on Windows Server 2008 R2.

Which version of UltraMon are you using?
I'm using SQL Server 2014 Management Studio as well, for me the buttons work fine on Windows 10 with UltraMon 3.3.0.

I would try upgrading to 3.3.0 first, should still be worth a try even though there's no fix for this or a related issue.

That's a free upgrade, you have a version 3 license which is valid for all 3.x releases.

I installed the latest version and it has not resolved the problem. I don't have the Ultramon buttons for Visual Studio or SQL Server Management Studio.

Unfortunately I can't test with the server versions of Windows and multiple monitors, but I'll test with Windows 7 (Server 2008 R2) or 8.1 (Server 2012 R2) and will get back to you.

I'm running Windows 7 and I have the same issue but only with Visual Studio 2013. I'm using SQL Server Management Studio 2012 and it has the window icons.

I have now tested on Windows 7, the buttons worked fine for me with SQL Server Management Studio 2014, but they were missing from Visual Studio 2015.
Forcing the window buttons for Visual Studio fixed the issue, to do this go to UltraMon Options > Compatibility, then configure settings for the application and check the option 'force window buttons'. I also had to set custom button spacing to 310 pixels.
