After updating Windows 10 to the Creator edition version 1709 the task bar on my primary monitor shows apps open on all four monitors. Would this be a Windows issue or an UltraMon issue?

Just a configuration issue, you can change this via UltraMon Options > Taskbar Extensions (change Mode to Standard), or via right-click on the taskbar, then select Taskbar Settings and adjust the 'Show taskbar buttons on' setting.

Unfortunately those settings are already configured the way you explain. I think Windows 10 Creator Edition has multiple issues and this is just one of many. I also found that every time I reboot, my taskbar which is configured as two rows always come back as one row and I have to drag it to two rows.

After hiding the Cortana icon on the primary task bar everything is back to normal.

Odd issue, haven't seen this myself so far with the 1709 release.
