With the show taskbar buttons on: taskbar where window is open (Taskbar settings in Windows personalization)
With Ultramon set to Remove start button from secondary taskbars - If the Cortana icon is displayed (round circle) it defaults to position two and overlays any icon below it. For example, if the second application open in the secondary monitor is IE, the IE icon appears behind the Cortana icon and you can't get to it. If you remove the Ultramon setting of hiding the start button in the secondary taskbar - the problem goes away.

Will be fixed in the next release, the only workaround is to disable the search/Cortana icon.
