The Colorgraphic Xentera GT 4 is a quad adapter but supports up to 4 DVI monitors at 1600x1200.

You can also check out the Matrox G200 series cards. They're available in PCI with either dual or quad DVI support.

FYI G200 does not support 1600x1200 DVI.
