I'm working on converting some PCs into appliances by replacing explorer.exe in Windows XP with a custom shell. The PCs will be used to display radiological images on dual TFT monitors, with IE spread over both screens. Ultramon works fine if Explorer.exe is running but if it isn't nothing Ultramon never seems to run. Is there a way around this? Can Ultramon be run as a service?

The Smart Taskbar might not work, and if the shell has no system tray you won't be able to access the UltraMon menu, but otherwise this should work fine.
What UltraMon features would you want to use with these systems?
Most UltraMon features require interaction with the user's desktop, this probably wouldn't work if UltraMon would be run as a service.
