Is there an easy way to force all applications to open initially on a given monitor? I thought I might be able to do this with UltraMon but I don't know.
If I close an application on a secondary monitor, when I reopen the application it always comes up on the secondary monitor.
Any way to force all applications to open on one default monitor?

UltraMon doesn't support this directly, you would need to configure custom position settings for each application.
To do this, right-click the application's shortcut, select Properties from the menu, then configure the settings on the UltraMon - Window tab as desired.

I don't know if this is the same issue are not, but I have my MS Outlook configured to use Word as my editor. I use outlook on my secondary screen and each time I go to reply it opens the reply document on my primary monitor.
Is there a way to have the reply document open on my secondary monitor?

Have you tried moving the message window to the secondary monitor and closing it there?
This works fine with the default configuration, the next time the message window will open on the secondary monitor, but I don't know if this also works if you use Word as the editor.
