Does anyone know the technical reason why certain monitors have to be set as primary in order for a system to work, with respect to Windows XP/2003 etc in particular? Is this a device driver flaw, and if so is it possible to devise some way around it?

Do you mean primary in the BIOS?
This is necessary for some older video cards, as far as I know it's due to the display driver needing access to the video card's BIOS, which on Windows 2000/XP and later is only possible if the card is the primary card in BIOS.
