I can't find a pattern to it, but certain programs I use a lot do not show up in the smart taskbar when I move the window to the secondary monitor.
Specifically, Visual Studio 2005 doesn't seem to switch - it remains on the primary taskbar.
I'm using UltraMon 3.0 B1 and Windows Vista x86.
Thanks a lot for UltraMon. It's a great program.

Does this only happen occasionally, or all the time?
Visual Studio 2005 works fine for me with UltraMon, the task button gets moved together with the application.
I have SP1 and the Vista update for SP1 installed for Visual Studio.
