Every now and then my taskbar buttons on the Smart Taskbar get messed up such that the buttons' Icons and Names don't map correctly to their respective applications' windows.
Because of this if I click on a taskbar button it will give focus to the wrong windows application.
Say for instance I have Outlook and IM running and there are Outlook and IM buttons in the Smart Taskbar. Normally clicking the Outlook button gives focus to Outlook. However, after it gets messed up, clicking on the Outlook button will give focus to IM instead and the IM button will give focus to Outlook.
It gets more screwed up if there are more applications open.
I am using Taskbar Shuffle 2.5 but I've been using that for several weeks and this problem seems to have cropped up in the last week or two.
I'm running Ultramon 3.0.2 in Windows XP.

Does this happen after the system or application has become unresponsive temporarily? If yes, the issue should be fixed in the next release.

I'll keep an eye out for patterns to reproducing this bug and see if your suggestion is the case. Thanks.
