I figure this problem would be pretty tough for you to troubleshoot, but I might as well report it anyway.
I'm using 3 monitors with Windows Vista Ultimate. First two are on a 9600gt card, and the third is on an 8600gt. I have the latest drivers from nvidia.
If Ultramon is running, every so often I'll go to the computer and discover monitor #2 has a weird static on the screen, and the screen goes black every few seconds. Monitor 1 and 3 never have the issue. I've swapped monitors, cables, etc, to no avail. The only way to make the problem go away is disable monitor #2 in the display settings, and then enable it again. Then everything is normal (until the problem returns eventually).
At any rate, if Ultramon isn't running, this problem never happens.

Sounds like a video card driver issue, but so far I haven't seen this myself, I have two 3-monitor systems with two Nvidia cards each (2x Quadro FX 3500 and 2x Quadro NVS 290).
For further troubleshooting, you could try if disabling a feature like the window buttons or Smart Taskbar fixes the problem.
