I'm using Windows Vista x64 and UltraMon 3.0.3 beta with two monitors. Normally, there's no problem, but when I run an old game in compatibility mode (WnXP SP2) and press Alt+Tab in that game to switch to the desktop, UltraMon crashes and the smart taskbar disappears.
That game is Total Annihilation. (yes, it's quite old one if you know.) It uses only the primary monitor and both monitors go in 256-colors mode while the game is foreground.
