Currently running 3.0.3 Beta on XP SP3, standard system using XP themes. I first noticed this problem when upgrading from 2.7.x to 3.0 beta 2.
Fairly regularly, one or more windows will lose their max/move buttons. Clicking in the space where they would have been does nothing. Context menu for the application still shows max/move menu items.
Resizing the window (e.g. drag right edge) restores the buttons.

I've seen that as well occasionally, not sure yet what exactly causes this but I'll keep an eye on the issue.

I've noticed that it happens often if the window is unresponsive for a bit and you mouse over the buttons.
In the case of VS, during startup I have a lot of plugins that initialize, and the VS window is essentially locked/unresponsive for ~30 seconds or so.
I'm now able to consistently reproduce it and it appears to be caused by a (temporarily) hung window. Ping me if you want steps to reproduce.

I'm seeing that as well with a solution which takes some time to load, initially the buttons get added but then they vanish. Will look into this for the next release.
