Just upgraded to 3.04 with no difficulties, but windows management icons are now missing from all Microsoft Office 2003 applications (Word, Excel, PowerPoint, Outlook and Outlook emails). But UltraMon drop-down still works if I right-click top of Office app windows.
Windows management buttons are working properly in Internet Explorer, Windows Media Player, system folders, and some non-Microsoft apps.
Any help is appreciated!

I've been able to reproduce this issue with Office 2003, will look into this for the next release.
