How can I horizontally mirror the extended screen?
I only get the option to horizontally mirror my main monitor, but not the extended one...

This is because UltraMon mirroring can't use the primary monitor as a mirror target monitor. If you want to mirror the other monitor, you would need to set it as primary first.

Maybe I didn't set it correctly...
I want the image on the second monitor (which shows the extended desktop) to be horizontally flipped.

Would you still want the extended desktop, but the image on the secondary monitor should be flipped?
This isn't possible via UltraMon, I'm also not aware of another way to do this.
