After my computer have returned from sleep mode, programs (that are maximized) on the monitors with smart taskbar 'ignore' the existence of them.
When a program is maximized, it simply 'hides' behind smart taskbar, using up the whole monitor, and not just the part without the taskbar.
However, activating autohide on the taskbar(s) and then deactivate it again solve the problem untill next startup from sleep mode.
I'm using Vista Ultimate x32 w/aero and ultramon 3.0.4

I have also had this problem for a long time. The SmartTaskbar reserves space for itself so that maximized windows are only able to use the part of the screen that the taskbar is not on, but at some point it just ignores it (perhaps after RDP into the machine).
Windows start going in front of or behind the taskbar, so that you can't see the taskbar, or you can't see their statusbar.

So far I haven't seen this myself, both after resuming the system from standby/hibernate or when logging in locally again after a remote session.
If you find out anything else, for example it only happens when another application is running as well, let me know and I'll look into it.
