When I opened PowerPoint 2003, the second screen would open up as 'Speaker Notes' while screen 1 would have the slide view. In Powerpoint 2007 the 2nd screen does not open separately - any ideas on how to get this functionality back?

This should still work fine, you can select the monitor to be used for the slideshow under Slide Show > Show Presentation On.
