You'll get this if an application stops responding, the buttons will then get drawn with a black border. Other applications should still work normally when this happens.
Regarding the crash: please save a crash dump when you get this the next time, to do this don't close the error message, then open Task Manager, select the Processes tab, right-click the application which crashed, for example UltraMon.exe, then select Create Dump File from the menu. Let me know once you have the crash dump.
