Many people have reported this issue with 3.0 beta. I have a 100% working repro.

1. find an app that uses a subwindow that is separate (not limited to the dimensions of the main window), modal, and has the bit set to appear on the taskbar. Good examples are Exchange Login dialog in Outlook 2007 and the "password input" dialog in PINS (http://www.mirekw.com/winfreeware/pins.html).
2. move the app to a secondary monitor and close it (so its starting location remembered)
3. reload the app
4. bring up the modal dialog
5. move the modal dialog to the primary monitor
6. dismiss the dialog

There will now be two entries on the secondary monitor's smart taskbar.

I'm getting different behavior with PINS, but have seen the duplicate task button issue with other apps occasionally (Remote Desktop, Outlook 2007 main window).

For me moving the password dialog to the primary causes the task button to get moved to the primary monitor (which also shouldn't happen), and after closing the dialog I have a single task button for the application on the primary monitor.

To be clear: after step 5 there are two taskbar entries, one on each monitor.

Running the same steps with Ultramon 2.x, after step 5 there is a taskbar entry on the primary monitor but not the smart taskbar. After step 6, the taskbar entry moves back to the monitor where the main window is.

Running the same steps with Smart Taskbar disabled, nothing happens. The modal dialog doesn't affect the taskbar at all.
