I'm running three monitors (2 ATI external PCI and 1 with the Intel on the motherboard). The two ATI are primary and secondary (monitors 1 and 2) and the Intel is monitor 3. When I try to slid icons over onto monitor 3, the icon graphic disappears and only the text is visible.
I'm using UltraMon on another machine with a similar setup and I've never seen this problem before.
Any ideas how to fix it?

I'm not sure what might cause this, but it's unlikely that this is related to UltraMon.
To verify this, check if you still have the problem without UltraMon running or installed.

Yes. We tried this and the problem only happens with UltraMon running. The monitor worked fine before we added another video card and installed UltraMon.
Any ideas how to fix?

Please try if the problem goes away if you make the following configuration changes under UltraMon Options:
- General tab, uncheck 'enable moving of maximized windows using the mouse'
- Smart Taskbar tab, uncheck 'use Smart Taskbar'
- Window Title Bar tab, uncheck both 'add ... button' options
- Window Menu tab, uncheck both 'add ... command' options
You should restart the system after making the changes.
